Sheriff's Office FOIA Request Forms

These forms are used by the Sheriff's Office and the Belvidere Police Department to handle complying with Freedom of Information Act requests. There are two forms - one for FOIA requests from the general public, and one for Commercial requests. Please submit the appropriate one. If you would like to submit a FOIA request, please carefully read the information below. Please be specific in the nature of your request in order for the FOIA officer to process your request efficiently.

METHODS TO REQUEST INFORMATION AND PUBLIC RECORDS: Request for inspection shall be made in writing and directed to the public body. Written requests may be submitted via personal delivery, mail, telefax, or other means available to the public. A public body may honor oral requests for inspection or copying. The public body may not require that a request be submitted on a standard form or require the requester to specify the purpose for a request, except to determine whether the records are requested for a commercial purpose or whether to grant a fee waiver. All requests for inspection and copying received by a public body shall immediately be forwarded to its Freedom of Information Officer or designee.

TIME PERIODS: Each public body shall, promptly, either comply with or deny a request for public records within 5 business days after its receipt of the receipt, and may obtain one (1) 5 day extension. Information sought solely for a commercial purpose has a lengthened response time of 21 days. Denial shall be in writing as provided in Section 9 of the Freedom of Information Act.

FEES: No fees shall be charged for the first 50 pages of black and white, letter or legal sized copies requested by a requester. The fee for additional black and white letter or legal sized copies shall not exceed 15 cents per page. Actual cost of copying for color copies. Cost for certifying a record not to exceed $1.00.
