Death Certificates

Death Certificates are available from the County Clerk's Office if:

• The death occurred in Boone County
• Death occurred between the years 1877 and March, 2009*

Cost of Certificates are as follows:

• $9.00 for first copy
• $4.00 for each additional copy

If you are requesting a death certificate by mail fill out the Death Certificate Form completely and send it along with the $9.00 fee ($9.00 for first copy, $4.00 for each additional copy), and a self-addressed, stamped envelope to:

 
Cash and personal checks from the State of Illinois only are accepted.
Otherwise, certified check or money order is required.  Credit and debit cards are not accepted.
 

*if death occurred after March of 2009, certificates can be obtained from the Belvidere City Clerk.
