Civil Union Certificates

Civil Union Certificates are available from the Boone County Clerk's Office if :

• Civil union occurred in Boone County
• Civil union occurred in 2011 or later

Cost of Civil Union Certificates

• $7.00 for the first copy
• $2.00 for each additional copy

If  you are requesting a civil union certificate by mail, fill out the Civil Union Certificate Request Form along with the $7.00 fee ($7.00 for the first copy, $2.00 for each additional copy), and a self-addressed, stamped envelope to:

Cash and personal checks from the State of Illinois only are accepted.  Otherwise certified check or money order is required.  Credit and debit cards are not accepted.
