TAXPAYER APPEAL FORMS

Residential, Commercial, Industrial, and Farm appeal forms are available to the public in a variety of ways.

The appeal forms are available at the Boone County Assessment Office, or they may be requested from the Illinois Property Tax Appeal Board via electronic mail or by calling or writing either the Springfield or Des Plaines office. The addresses and telephone numbers are located at the top of this page. In your request, please be sure to include your full name, address, day and evening telephone numbers, and the type (residential, farm, commercial or industrial) and quantity of forms you are requesting.
