TAXPAYER APPEAL FORMS

Residential, Commercial, Industrial, and Farm appeal forms are available to the public in a variety of ways.

The appeal forms are available at the Boone County Assessment Office, or they may be requested from the Illinois Property Tax Appeal Board via electronic mail or by calling or writing either the Springfield or Des Plaines office. The addresses and telephone numbers are located at the top of this page. In your request, please be sure to include your full name, address, day and evening telephone numbers, and the type and quantity of forms you are requesting.

If you have Microsoft Word loaded on your computer, you can download a version of the appeal forms which will allow you to fill them out right on your computer.

IMPORTANT NOTE: It will still be necessary for you to mail your appeal form or hand deliver it. Forms which are faxed or e-mailed will NOT be accepted. The original signature of the appellant or attorney is always required on the appeal form.

PLEASE NOTE:

If you do not have Microsoft Word, you must use the Adobe Acrobat Portable Document Format version of the form; otherwise, the appeal form will be distorted, and the PTAB will be unable to docket your appeal.

Since these are large files, we recommend that you right-click the file and use the "save as" or "save link as" function provided by your browser to save the file to your computers hard drive, or a removable disk. This will allow you to work on your form off line.
