Birth Certificates
Birth Certificates can be obtained from the Boone County Clerk's Office if:
- Birth occurred in Boone County, IL
- Birth occurred in 1877 or later
Who can receive a Birth Certificate?
- The individual named, if 18 years of age or older.
- A parent, if the parent's name appears on the Birth Certificate as mother or father.
- A Legal Guardian, with the Court Order stating said guardianship.
- The person's legal representative.
- A department of the State or Federal Government
- Any person upon order of a court
What ID is required to receive a Birth Certificate?
ONE of the following:
- A valid Driver's License
- State ID Card
- Naturalization Certificate
- U.S. Military ID Card
- U.S. Immigration Card
- Selective Service Card
- Illinois Public Aid ID Card
or TWO of the following if one of the above not available:
- Voter Registration Card
- Bank Statement (within 90 days)
- Vehicle Registration Card
- Traffic Ticket
- School Identification Card
- W-2
- Utility Bill (within 60 days)
- Employee Identification Card
- Resident Alien Card
- Active Insurance Policy (At least one year old)
- Checkbook or Savings Book (showing name & address)
How can a person get a Birth Certificate?
- In Person, at the Boone County Clerk's Office, 1212 Logan Ave, Belvidere, IL 61008. Monday - Friday, 8:30am to 5:00pm.
- By submitting a request by mail.
- By submitting a request online. Additional fees apply.
Cost of Birth Certificate
- Years 1916 to Present: $12.00 for 1st copy, $4.00 for each additional copy
- Years 1877 to 1915: $25.00 for 1st copy, $4.00 for each additional copy
Cash and personal checks from the State of Illinois accepted. Otherwise, certified check or money order is required. Credit and debit card are NOT accepted unless submitting requests online.
Civil Union Certificates
Civil Union Certificates are available from the Boone County Clerk's Office if :
- Civil union occurred in Boone County, IL
- Civil union occurred in 2011 or later
ONE of the following:
- A valid Driver's License
- State ID Card
- Naturalization Certificate
- U.S. Military ID Card
- U.S. Immigration Card
- Selective Service Card
- Illinois Public Aid ID Card
or TWO of the following if one of the above not available:
- Voter Registration Card
- Bank Statement (within 90 days)
- Vehicle Registration Card
- Traffic Ticket
- School Identification Card
- W-2

Cost of Civil Union Certificates
- $12.00 for the first copy
- $4.00 for each additional copy

Death Certificates
Death Certificates are available from the County Clerk's Office if:
- The death occurred in Boone County, IL
- Death occurred in 1877 or later.
ID is required to obtain a Death Certificate. Please provide:
ONE of the following:

- Naturalization Certificate
- U.S. Military ID Card
- U.S. Immigration Card
- Selective Service Card
- Illinois Public Aid ID Card

- Years 1916 to Present: $16.00 for 1st copy, $8.00 for each additional copy
- Years 1877 - 1915: $29.00 for 1st copy, $8.00 for each additional copy

Marriage Certificates
Marriage Certificates are available from the Boone County Clerks Office if:
- Marriage occurred in Boone County, IL
- Marriage occurred in 1838 or later
ID is required to obtain a Marriage Certificate. Please provide:
